What is an itemized bill?

A summary statement shows a total amount due, maybe grouped by department. An itemized bill breaks down every individual charge with procedure codes (CPT/HCPCS), descriptions, quantities, and prices. Itemized bills are what you need to identify specific errors.

"Total charges: $7,400" — not enough detail to identify errors.

Itemized Bill

Line-by-line: CPT 99223 — $4,850, CPT 85025 — $340, etc.
